Per the item description, when activated, the flamethrower "Can deal fire damage to targets struck in melee".
Despite extensive testing with the pilot-light activated, I have yet to see a successful melee hit deal any fire damage to a target.

Flamethrower still does not deal damage while lit and in melee, as shown in screenshot. All damage is physical and subject to armor resistance. Not seeing any bonus damage from fire; none on Char-sheet, weapon card, or targets. Attachments:
