This does not happen every time, but it occurs quite often. It's not related to the recent instant active change; it has also occurred prior to change.
In this state, it's only possible to shut it off by filling it with fuel and turning it off. Otherwise, it will remain active even if put in the character's inventory.

Also occurs with the Electric shield.
(would not be surprised if all the melee electric have the issue, gloves included)
This does not occur with fire/acid/cold
But it seems to turn off after ~ a long time if this occurs. Usually after a long afk, for example.

Now that I've gone back to using my whips again, I've been gathering some data on this issue:
James has Gadgeteering 4, which improves whip duration but should not be a sufficient efficiency improvement to produce this bug.
I've noticed that the whips bugging out is inconsistent, and can be more reliably (but by no means perfectly reliably) reproduced by recharging the whips when the ability-button is greyed out and the elemental effect is active. The bug is visible when active whips (or presumably other equipment) are unequipped and no "a_item has been turned off." appears.
As an experiment, I abused my whips in the arena until both were bugged, recharged both with a Recharge grenade, and attempted to run a comparison: Both Test and Control whips were precharged at the same time with a Recharge Grenade, and checked after 50 minutes by manually fueling with Batteries. Control: 0 fuel used; when I attempted to refuel it the electrical ability turned off (no feedback in Combat log). Test: Icon greyed out after ~35 confirmed hits, continued to be active until I stopped testing 155 hits later, but probably could have gone on longer. 16 fuel consumed when recharged; also turned off without Combat Log feedback when refueled manually.
Follow up testing: Test whip was then reactivated and used for more fighting in the arena: Icon greyed out after 21 hits and a little under 3 minutes but whip continued to be charged. Test whip was recharged (consumed 16 batteries) and unequipped; no "Whip was turned off" message occurred. Both whips were then activated and deactivated. Whips were then activated and unequipped, which triggered the "Whip was turned off" messages for both in the Combat Log.
Test whip was reactivated and used in combat, whereupon it lasted for 6 hits and slightly over 2 minutes, before turning off with a note in the Combat Log.
Current source of the issue is inconclusive. However, my hypothesis is that the fuel-check script has some condition related to fuel consumption where it 'forgets' if an item is active and doesn't deactivate the ability. I have seen my aspergillum (cryosine) last longer than normal a time or two, but whips are much easier to glitch in this matter, and I suspect it has to do with the difference in fuel consumption rates.

Ironically, after telling Kit that I'd never seen it occur on anything besides an electric device...it occured with a Aspergillum cryo weapon.
Twf with 2 devices. I went down exactly at the moment where the devices were to run out of fuel, one made the deactivate and the other remained active. Once I revived (the battle was won by the Npc), the remaining device was not deactivated and remained on.
So, likely the deactivate script is being bypassed at rare times or maybe the code exits the nested check or something.

I have an inefficient fix for this that I'll add next update. If it works I'll eventually revert it and try to make a better one.

I have managed to replicate this issue even after the fix.
Based on my testing, I suspect that this bug has to do with a fuel change or equip change that occurs when the current fuel is less than 1. This would be about an 8 second window on a normal electrical whip and up to 16 seconds at Gadgeteer V (assuming 0.125 fuel/sec is still accurate), which would explain the relatively higher frequency of the whip in particular bugging compared to other fuel consumers. While I do not know the specific instance, sequence that initiated it was as follows:
Whips were uncharged, charged whips before testing. All recharging of whips was done using Recharge Grenade rather than batteries.
Entered match, turned on both whips. Whips ran out during match, recharged and turned back on. Lost match, unequipped whips, both turn off messages fired.
Entered next match, turned whips on. During match, recharged both whips before they needed recharge. Lost match, unequipped both whips, turn-off messages fired.
Entered next match, turned whips on. Won match, whips were charged but grayed out shortly before victory. Unequipped whips, neither turn-off message fired.
Entered next two matches, whips remained grayed out and did not need turning on or charging.
Recharged whips, unequipped, no turn-off message fired. Manually turned off whips, turned on and unequipped; turn-off message fired.

Elemental weapon mods now turn off a few seconds after activation. Consistently.

Bumping this. My Oil Canisters are now sticking pretty consistently, for the past few days maybe half the time. I've never had them stick before that I recall. It might be coincidence, but I have some doubts.

Any details? The duration on oil canisters is quite long if you aren't hitting things with them, and it would be much easier to debug this if you had information about what happened to the icon, how much fuel it took on the next refueling, whether it was still dealing fire damage in the bugged state, etc.

The icon turns dark as if the item was out of fuel. Re-fuelling the item takes the the normal amount of fuel (5). It continues dealing fire damage as normal, and the duration is something like 2 hours.... though it does -eventually- turn off.
EDIT : Today it persisted post server update.

I would suggest reverting to the original script that was in place when I first reported this.
My reasoning:
-Currently with 0-2 refills, it is guaranteed to have perpetual elemental weapons (oil canister, aspergillum, electric whip, etc). Perpetual = hour+ duration. This is noticed from my limited use of late.
-at the time of OP, I would see whips go perpetual from time to time, but it was extremely rare for aspergillum and cannister. I only ever saw it once with aspergillum - which might have been due to the tweaks - and never with canister.
So the original script was ok, considering. (In reference to the perpetual bug)

Not reverting, it won't matter either way once the bug's fixed.
I found a problem involving items that are turned off immediately after dealing damage with a melee attack, if they're rolling over to "persistent" during combat then this is likely the main cause of the bug, if not the only one.
