Put off fixing it because I wanted to do a rewrite/nerf that follows some of the other suggestions I've seen. The new version should work with abilities by default but if it doesn't I can think of a few ways to add it.

Flanking is definitely not adding the +1 from the counterweight mod. The Nwn2 vanilla +2 bonus is added when attacking from a successfully correct rear angle.
Counterweight also does not apply to a prone target with Coup de Grace III, which provides a flank attack on a KD'd target. The +4 KD melee bonus is added. I have not seen the +2 flank bonus added in this case, but this may just be the finicky-engine melee positioning.
(I don't have the wider flank angle feats and thus can not comment on that situation)

I'm pretty sure flanking bonus is not supposed to apply to KD'd targets even with Coup De Grace, because flanking bonuses (on auto-attacks) are hardcoded. (And don't apply on Knocked Down targets). Pretty sure it only triggers the bleed damage.

The new counterweight code is meant to increase your AB when your PC is attacking a creature who you flank. You should expect to see it directly on the character sheet instead of as a +3 difference on attacks.

The only feedback I have seen is from the combat log, which is only the vanilla +2 flank bonus. I'm not able to observe any AB changes in the character sheet during combat. (might be a refresh thing)
It would be useful to have a black text msg in the combat log announcing this bonus, when it procs. With this msg it might be easier to look closer at the intended effect.

Worked fine on a test PC. I got a pet and a longsword with a counterweight, had my pet initiate the fight, then went behind and started attacking. My AB went from +9 to +10, and returned to +9 when the creature turned from my pet back to my PC. Let me know if you see anything that isn't in line with that, and if so what other details might have been different.

Ok, I've seen up/down of +1 to the character sheet. I just kept the sheet open and the end result is +3 when attacking from the appropriate rear angle. However, I wasn't using a counterweight weapon. Probably needs a verification that the mod is there. (It very well could have been that I just didn't watch the sheet carefully enough when I was in fact using a counterweight weapon - as I tend to move around a lot. I'll look for that when I'm running with the counterweight mod). I must have just been briefly popping the sheet open and not paying tight attention to the exact number variance on the char sheet. EDIT: I also see +1 boost when in block mode upon a successful riposte - which stays until a failure to block, I think. I have no special block feats that could possibly account for this.

Halberd with 2 mods: Magnetic Attractor and Heel Catcher.
(It had counterweight prior but was changed - I doubt this historical change should impact the issue, but I mention this detail for completeness)

Couple of questions:
1. is the +1/-1 shift associated with the flank feat? 2. what is the condition that provokes/removes the +1?
^ I ask these as the bonus is actually on a lot, and that may be another reason that I never noticed.
Quite often what appears to be a normal-facing melee hit [on a mob] will initiate this bonus and it stays on until I am hit in melee?

Looks like the magnetic attractor was doing it, fixed for next update.
The +1 is not related to any flanking abilities and is removed if the target turns towards you.
