so, slightly different situation than previous instances I've seen here, as no DM interactions were involved. Yesterday, a group of us were attempting to run Desert with horses, and we dies a few times doing things that didn't work. Today, we had a server reset for update. A group of us were killing trolls and trees outside the waterfall, but I didn't actually die at all. We finished for the evening, came back to town. I crashed transitioning from the Wards to the Trade Quarter. When I restarted the game and logged back in, I was greeted by the screenshot: being told I was killed before loading in, that I was tied to the Nefarian re-spawn point, and that there was no valid death-rule, and my jet-pack was unequipped. My incompletely informed supposition is that when I crashed, the character-state I had yesterday was applied somehow, unequipped my jet-pack and fuel-belt and sent me to the Newport re-spawn location. My inventory was untouched otherwise, and the jet-pack might just have been the visual effect. Attachments:

The biggest piece of information is how you got up the last time you went down, since that's the thing responsible for ticking the box saying you didn't log off dead.

oh, right, yes. I *did* use a Autodefib in the troll caves that fired. in the desert, I died twice (resapwned at newport) and autodefibbed once (which posted HCR2 state message).
The autodefib from the run in troll-caved did NOT post an HCR2 state message when it fired.
