Last night I was in an event where Zesiro was killed with fire in a DM house, which caused her to fall unconscious and bleed to death (mechanically) DM healed me, we went on with the event, everything was good, then I had to go to bed and logged after being ported back into the city.
This morning I logged in any my character was just outside the graveyard, and had been hit with a death penalty (20% gold). I'm not sure what happened for sure, but this is it described to the best of my ability.

Can you give more info about how you got to the dm event? It looks like hcr2 thought you logged off after dying and used the last saved death rule on your character. I can probably fix it by writing a death rule for dm events and editing the dm heal commands.

Our group moved from the trade quarter to the docks, then in and out of the chesterfield detectives safehouse. We made our way down to the south eastern docks, were ported into a house, we fought a bunch inside, Zes went unconscious and bugged out to burning damage, so she triggered another death script where she bled to -10 and stayed there. Fuzz gave me a DM rest, or set my HP to 100% I'm not sure which, then set me to almost dead. Recovered, got up continued through the event, the RP probably went on for another hour or so back through the docks and trade quarter, and into a jail cell. There was no more combat after that part though, eventually I was tired, got a port out into the trade quarter then logged out.
When I logged in, I was at the entrance of the graveyard as though I'd just died inside, my combat log said I'd been healed, applied death penalty, and that I'd lost 200 bucks.

Found something that seems to be a very likely cause for this - the system will clear out stale "death rule" variables if you get healed, revived, or enter a safe area from a dungeon.
