I've noticed that right-clicking on any gear items in the hot-bars pulls up the menu, and leaves the cursor hovering directly over the "Deconstruct" button. This may have resulted in one of my spears disappearing when switching to a melee weapon in combat.
I've no idea what the feasability of it might be, but removing the deconstruct option from the hot-bar might be an idea to follow up on. Alternatively, having deconstruction take an extra confirmation step or a toggle to turn deconstructing on and off (perhaps in the player menu box?) could prevent further unfortunate incidences with otherwise irreplaceable gear. Otherwise, the only people not at risk at eating their own gear are the people who can make it. Simplest might be to re-arrange the menu buttons so that "Improve" is on the bottom, since it seems overall safest for retention (in the event of accidental right-left-click, it changes a page in the char menu, instead of deleting an item or dropping it).
On a good note, deconstruction progression appears to be working admirably. I'm already 17% of the way to one of the mods I require to re-produce the spear, and 52% on the other. Diving into my stash has verified that they do lock out correctly if you have learned all mods an item can 'teach'.

I'm just going to add an "Are you sure?" box when you hit deconstruct, it's a couple lines of script to make that work.
