This is quite probably a bug, but as I recall, the lack of a death-rule in city areas during a PVP encounter left one charachter KOed in the middle of the Trade Quarter until I found them and hit them with a defibrillator.
I would like to suggest a solution fo being 'stuck unconcious' in city areas, in that a different death-rule gives a charachter a timer of some number of minutes of being down (to prevent abuse during PVP and to give other characters an attempt to revive them through RP), and once that timer finishes to give the KOed character 1 HP and a bunch of cumulative damage (so max HP is very low).
This would mitigate the potential of griefing by killing a player and leaving them stuck in the city without any way of getting up, while attempting to address the potential for RP to occur around a KOed person. If they attempt to rush back into a fight after waking up with severe blood loss on the cobblestones, they probably won't survive long without serious medical assistance, but can drag themselves to a rest location and recuperate without being excessively locked-out of game-play.

Currently there's a system in place that replaces death by a knock down if the killer is a PC and the area has no other death system in place, so the city stuff should be fine unless it's DM-initiated, in which case the DM needs to clean up afterwards.
